Calvert Manor Healthcare Center is a fairly large 144-bed nursing home in the small town of Rising Sun in Cecil County Maryland that has been in business for over 70 years.

Many these large nursing homes in rural areas are proving grounds for nursing home negligence and abuse lawsuits. Our lawyers get calls with horrific reports of neglect and unimaginable mistakes from so many for-profit nursing homes that sound just like Calvert Manor.

But this narrative does not describe Calvert Manor. By any objective criteria, this is a very good nursing home in a beautiful setting in a very nice area of our state. It blows past the two other major nursing homes in Cecil County - Laurelwood Care Center at Elkton and Elkton Transitional Care Center - by almost every objective measurement.

The idea of a nursing home being a for-profit corporation might give some people pause, perhaps concerned that profitability might take priority over quality. That does not seem to be an issue for Calvert Manor. Although technically a limited liability company (LLC), Calvert Major gets top Medicare report ratings: 4 of 5 stars (above average) for staffing and quality measures, 5 stars for health inspections, and a 5-for-5 star rating (much above average) overall. No doubt, a great many nonprofit organizations would envy these scores.

Staffing is a big deal. Mistakes happen and care suffers when a nursing home is not properly staffed.

Although larger than average in terms of population (132 residents at last official count, versus state and U.S. averages of 107.5 and 86.2, respectively), Calvert Manor has shown that it can maintain high quality even while operating on a larger-than-average scale. The last standard health inspection, in September, 2015, turned up a total of two health deficiencies, besting the state and national averages (Maryland, 11; U.S., 7.1).

Calvert Manor got an overall staffing rating of 4-for-five, but 5-for-5 in the category of registered nurses. (Years ago, there were concerns about the staffing at this facility.) Although almost the same as the state and U.S. in terms of RN time per resident (63 minutes per day, versus 59 for Maryland, 51 across the U,S.), Calvert Manor promises that "RNs are on duty and onsite 24 hours/day." A registered nurse is always there, if you need one.

This facility is a bit lower than the Maryland and U.S. averages on total nurse hours per resident per day, certified nursing assistant time, and physical therapist time. It is considerably lower on time spent by LPNs and LVNs (licensed practical nurses, licensed vocational nurses): 33 minutes, versus 52 minutes statewide, 51 minutes for the U.S. Calvert Manor's staffing ratings are fairly comparable to state and national averages, but this facility gets that overall staffing rating of 4 of 5 stars: its 5-star rating for RNs may be the difference.

Calvert Manor is average or a bit below average in three measured categories for short-stay patients. It is a bit better than state and national averages on successful discharge rates. It is significantly better than average on helping residents improve their functionality; on preventing bed sores; on administering flu and pneumonia vaccines; and on limiting the use of antipsychotic medications. Because nursing home lawyers see so many bed sore cases, the data for that markers is particularly interesting to us. There is such a correlation between the overall quality of a nursing home and how it handles bed sores. Here, we see that Calvert Manor seems on top of the bed sore issue, at least for the majority of patients.

For long-stay residents, this facility beats the averages on vaccinating for flu and pneumonia: Calvert Manor does it more. In other categories, Calvert Manor excels by coming in less: fewer urinary tract infections; fewer cases of excessive weight loss; less ongoing catheterization; fewer residents with symptoms of depression; and no use of physical restraints.

Calvert Manor comes in around the state and national averages on long-stayers' incidence of pressure ulcers and resident reports of pain. It has a slightly higher rate of resident falls with injury; a slightly lower-than-average score on maintaining patient mobility; and slightly higher-than-average use of antipsychotic meds. This facility's long-stay residents have an incontinence rate of 61.6 percent, very close to the state average of 58 percent. But in this case, where the lower number is better, both Calvert Manor lag behind the national average of 46.6 percent. The facility has had no federal fines or payment denials in the past three years.

Though its scores are not perfect in every category, its overall 5-for-5 rating would suggest that the operators of this facility have been doing a lot of things right.
