Citizens Care and Rehabilitation Center of Frederick is a large, 170-bed nursing home in Frederick, Maryland. This nursing home is managed by Auora Mid-Atlantic LLC, a Millersville company that owns or manages a number of nursing homes in Maryland.
This nursing home has an interesting history. The Medicare ratings for Citizens Care crashed from four stars to two stars in 2015 based on health inspections (here is one that was pretty brutal), staffing, and quality measures.
Auora had just bought the company so there was a lot of fingerpointing as for who was responsible for this fall from grace.
To make matters worse, the rehabilitation group in the facility, RehabCare, had been giving unreasonable amounts of therapy to the facility's Medicare patients until the Justice Department stepped in and gave them a whopping fine. The train was arguably falling off the rails.
Looking at these nursing homes, the path is bad to worse. But Citizens Care went the other way. Frederick County Government retained operations in 2016. Auora Mid-Atlantic, which apparently stayed on as the management company, found some way to turn it around. In 2017, Citizen Care got a 5-star overall rating from Medicare. It still had a health inspection that was nothing to brag about, but there is no question this facility is doing much better. Crazy story.
In spite of the problems it had, we were unable to find any recent nursing home abuse or neglect lawsuits against Citizen Care.
